A technique with great advantages
In the past, when your surgeon recommended gallbladder surgery, perhaps you thought about the experience of a family member or friend who had had this surgery years ago. These people had a large incision and had probably had a lot of pain after surgery. They were in the hospital for a week and did not return to normal activities for about six weeks. You may be worried about a similar experience. Perhaps you can't afford to be away from your job, and you're worried about not being fully functional at home for a month or more.
Today there is an innovative technique with great advantages. The medical name for this procedure is Laparoscopic Cholecystectomy. Today, most gallbladder surgeries are performed laparoscopically.
- Instead of an incision of 13 - 18 cm, the operation requires only four small openings in the abdomen.
- The patient usually has minimal postoperative pain.
- The patient usually experiences a faster recovery than patients operated with traditional gallbladder surgery. Most patients return home within a day and enjoy a quick return to normal activities.
- How is laparoscopic gallbladder removal performed?
- Under general anesthesia, the patient will be asleep during surgery.
- Using a cannula (thin tube) the surgeon enters the abdomen in the navel region.
- A laparoscope (a thin telescope) connected to a special camera is inserted through the cannula, giving the surgeon a magnified image of the patient's internal organs on a television screen.
- Other cannulas are inserted to allow your surgeon to delicately separate the gallbladder from its adhesions and remove it through one of the openings.
- Many surgeons perform an X-ray, called cholangiography, to identify stones which may be located in the bile duct or ensure that anatomical structures have been identified.
- If the surgeon finds one or more stones in the common bile duct, he or she may remove them using a special endoscope, may decide to remove them later using another minimally invasive procedure, or may convert it to open surgery to remove all the stones in this surgery.
- After the surgeon has removed the gallbladder, the small incisions are closed with a stitch or two, or with surgical tape
- What if surgery cannot be performed laparoscopically?
In a small number of patients the laparoscopic method is not possible due to the inability to visualize or manage the organs effectively. When the surgeon decides that it is safer to convert laparoscopic surgery to open surgery, this is not a complication. This decision means good surgical judgment. Some factors that increase the risk of conversion to open surgery include obesity, a history of previous abdominal surgeries that cause dense scarring, or bleeding during surgery. The decision to execute an open procedure is a judgment decision made by your surgeon before or during surgery. The decision to convert to any open procedure is based strictly on patient safety.
- How long will I stay in the hospital?
Most patients return home the day after laparoscopic cholecystectomy. Some patients may even return the same day of surgery, compared to five days after a traditional open procedure.
- When will I be able to return to work
Most patients can return to work within seven days after the laparoscopic procedure. Of course, this depends on the nature of your employment. Patients with clerical or desk jobs usually return within a few days, while those with manual labor or heavy lifting may take a little longer. Patients undergoing traditional procedures cannot return to normal activities for four to six weeks.
Gallbladder lithiasis is one of the main pathologies in national health resulting in more than 500000,40 cholecystectomies per year. Most surgeries are for symptomatic gallbladder lithiasis and more than 90% of cholecystectomies are performed laparoscopically. Other forms of treatment are palliative and non-curative.
